Contractor Company & License Information

Note: California law requires all contractors to be licensed by the Contractors State License Board (CSLB) for any construction project valued at $500 or more in combined labor and materials costs.

MTZ Building Solutions requires all subcontractors to maintain:

  • Valid California Contractor's License
  • Workers' Compensation Insurance
  • General Liability Insurance (minimum $1M per occurrence)
    • Current Contractor's Bond

As part of the application process for licensure with the Contractors State License Board (CSLB), I understand that I am required to submit a full set of fingerprints for the purpose of conducting a criminal history record check.

These fingerprints will be used to obtain criminal history information from the California Department of Justice and the United States Federal Bureau of Investigation.

This information will be used to determine my eligibility for licensure under the criteria set forth in the California Business and Professions Code, including Section 480.

Failure to comply with this requirement constitutes cause for disciplinary action by the CSLB.

In addition, I acknowledge my rights under California and Federal law, including the following:

• I have the right to receive a copy of the background check report.

• I have the right to dispute any inaccurate information in the report.

• I have the right to receive a notice of any adverse action taken against me based on the report.
